Ankle Sprains

Ankle sprains are among the most common musculoskeletal injuries, affecting people of all ages and activity levels. They can occur during sports, exercise, an accident, work, or routine daily activities. While many ankle sprains are considered minor injuries, some can involve significant damage to the ligaments that support the ankle joint and may lead to ongoing pain, instability, or recurrent injuries if not properly evaluated. An ankle sprain occurs when one or more ligaments in the ankle are stretched beyond their normal limits or torn. Most ankle sprains involve the ligaments located on the outside (lateral side) of the ankle. These injuries commonly occur when the foot rolls inward unexpectedly, causing excessive stress on the outer ankle ligaments. Less commonly, sprains can affect the ligaments on the inside of the ankle or the ligaments that connect the two lower leg bones above the ankle joint, known as syndesmotic or “high ankle” sprains.

The severity of an ankle sprain can vary considerably. Some injuries involve only mild stretching of the ligaments, while others result in partial or complete ligament tears. The extent of ligament damage often influences the severity of symptoms and the recovery process.

Ankle sprains typically occur when the foot is forced into an unnatural position that places excessive stress on the supporting ligaments. This sudden movement can cause the ligaments to stretch or tear.

Sports and recreational activities are among the most common causes of ankle sprains. Activities that involve jumping, pivoting, cutting, or rapid changes in direction place increased demands on the ankle joint. Basketball, soccer, football, volleyball, tennis, and trail running are frequently associated with ankle injuries.

However, ankle sprains are not limited to athletes. Everyday activities can also result in injury. Stepping off a curb awkwardly, walking on uneven ground, slipping on wet surfaces, or missing a step on a staircase can all cause the ankle to twist unexpectedly.

Several factors may increase the likelihood of sustaining an ankle sprain. Previous ankle injuries are one of the strongest risk factors. Individuals who have experienced an ankle sprain in the past may be more likely to suffer another due to residual instability or weakness. Poor balance, muscle weakness, inadequate footwear, and participation in high-impact sports may also contribute to increased injury risk.

Certain environmental conditions can further raise the risk of ankle sprains. Uneven terrain, poorly maintained playing surfaces, loose gravel, and slippery conditions can all increase the likelihood of losing footing and injuring the ankle.

The symptoms of an ankle sprain can range from mild discomfort to significant pain and difficulty walking. Symptoms often develop immediately after the injury, although swelling and bruising may become more noticeable over the following hours.

  • Pain is typically the most common symptom. The pain may be felt along the outer side of the ankle, although its location depends on which ligaments have been injured. The severity of pain often corresponds to the extent of ligament damage.
  • Swelling frequently develops soon after the injury.
  • Bruising is another common symptom. Torn blood vessels within the injured tissues can cause discoloration that may appear blue, purple, or yellow as the injury heals. The bruising can be seen around the heel or even the toes, depending on the severity of the sprain.
  • Many individuals experience tenderness when touching the injured area. The ankle may also feel unstable or weak, particularly during weight-bearing activities.
  • Difficulty walking is common, especially with more severe sprains. Some people may be able to walk with only mild discomfort, while others may find it difficult or impossible to bear weight on the affected ankle.
  • In certain cases, patients report hearing or feeling a “pop” at the time of injury. While this does not always indicate a severe injury, it may suggest significant ligament damage and should be evaluated by a medical professional.

Proper diagnosis is important because ankle sprains can share symptoms with fractures, tendon injuries, cartilage damage, and other conditions affecting the foot and ankle.

Diagnosis typically begins with a detailed medical history and physical examination. Your orthopedic specialist will ask how the injury occurred, when symptoms began, and whether previous ankle injuries have occurred. Understanding the mechanism of injury often provides important clues regarding which structures may have been affected.

During the physical examination, the ankle will be carefully inspected for swelling, bruising, deformity, and areas of tenderness. The physician may gently move the ankle through different positions to evaluate ligament stability and determine which structures are involved.

Specialized examination techniques may be performed to assess the integrity of specific ligaments. The physician will also evaluate your ability to bear weight and examine surrounding structures, including the foot, lower leg, tendons, and nerves.

Imaging studies are often used when necessary to confirm the diagnosis or rule out other injuries. X-rays may be ordered to evaluate for fractures or other bone abnormalities. Although ligaments do not appear on standard X-rays, these images can help determine whether a broken bone is contributing to the symptoms.

In certain situations, advanced imaging such as magnetic resonance imaging (MRI) may be recommended. MRI provides detailed images of the ligaments, tendons, cartilage, and other soft tissues surrounding the ankle. This can help identify the extent of ligament injury and detect associated damage that may not be visible on X-rays.
